Consultation

- We will sit together and go over your medical history, goals and outline a plan together moving forward that is individualized to YOU and your specific goals, limitations and resources.
3 Month Subscription
$600
- We will meet at our initial consultation, outline a plan tailored to your goals and then meet every two weeks to monitor progress, changing course as needed. You will have access by email, text, and phone whenever you have questions. This will be direct access to Dr. Carrillo since there are no nurses, medical assistants, or other support staff – you will get THE DOCTOR, each and every time.
6 Month Subscription
$1100
- This package follows the same option as the 3-month subscriptions, however after the first 3 months, we will meet as frequently as you feel is necessary to keep you on track, managing the tools we go over together. This can be every 2 weeks, every month or even less often depending on how things are progressing. All the same options are available as the 3-month option, but you lock in a lower rate for a longer duration.
12 Month Subscription
$2000
- This option is an extension of the 3- and 6-month option but with a reduced rate for a longer duration with all the same access as the previous subscriptions.
Extra Add on Options
Pantry Sweep - $200
We go through your fridge and cupboard and remove/discuss the foods that won’t help you on this journey and those that will.
Grocery Shop - $200
We grocery shop together, discuss labels and find options that fit your tastes but are healthy choices.
Discounts
- Discount plans for couples/families – 20% off total subscription price
- Medical providers – 40% off total subscription price
Penalties
- No show fee – $50 (require 24 hr notification to avoid penalty)
- Cancellation – if you choose to cancel your subscription before the agreed term has ended, you are responsible for paying the remainder of the contract or differences between services already rendered and payments already received.
